Sherwood Medical Clinic is a new physician-owned family medicine clinic opening in September 2026 in northwest London, Ontario. Our team of nine family physicians provides comprehensive primary care to more than 10,000 patients. We are seeking an experienced and motivated Registered Practical Nurse (RPN) to join our team. This role combines direct patient care, clinical support, triage responsibilities, staff education, and clinic operations support. The successful candidate will work closely with physicians, administrative staff, and patients to help ensure the delivery of safe, efficient, and high-quality primary care services.

Schedule

● Full-time permanent position

● Monday to Friday schedule

● Participation in a rotating evening clinic schedule

● Approximately one evening shift per week until 8:00 PM

● Participation in a rotating Saturday clinic schedule

● Approximately one Saturday every five weeks (12:30 PM – 4:00 PM)

● Statutory holidays off

Position Summary

The Registered Practical Nurse (RPN) is responsible for providing clinical nursing support within a busy family medicine clinic. Responsibilities include patient assessment, clinical triage, injections and immunizations, specimen management, chronic disease support, preventative care initiatives, procedure assistance, staff education, and support of clinic quality improvement activities.

The successful candidate will serve as a clinical resource to both physicians and staff and will help ensure patients receive timely and appropriate care.

Key Responsibilities

Clinical Nursing Duties

● Perform nursing assessments within RPN scope of practice.

● Obtain and document patient histories and clinical information.

● Obtain and document vital signs including:

o Blood pressure

o Pulse

o Respiratory rate

o Temperature

o Oxygen saturation

o Height and weight

● Perform patient assessments and identify abnormal findings requiring physician review.

● Administer injections, immunizations, and other delegated treatments.

● Provide patient education regarding medications, treatments, preventative care, and chronic disease management.

● Support preventative care initiatives, including screening and immunization programs.

● Assist physicians with office procedures, including:

o Joint injections

o Skin procedures and biopsies

o PAP testing

o Wound care

o Minor surgical procedures

● Perform dressing changes and wound care as appropriate.

● Coordinate follow-up care and patient monitoring.

● Assist with chronic disease management programs.

● Support vaccine inventory management and cold-chain monitoring.

● Participate in infection prevention and control activities.

Triage Responsibilities

● Assess and triage patient concerns by telephone and in person.

● Determine urgency of patient concerns using established clinic protocols and nursing judgment.

● Identify patients requiring:

o Emergency Department assessment

o Same-day appointments

o Semi-urgent appointments

o Routine appointments

● Escalate complex or urgent clinical issues to physicians appropriately.

● Document triage assessments and recommendations accurately within the EMR.

● Assist in developing and refining clinic triage processes and protocols.

Staff Education and Clinical Leadership

● Assist with onboarding and orientation of new clinical and administrative staff.

● Provide training and mentorship related to clinical workflows and procedures.

● Support staff competency development regarding:

o Infection prevention and control

o Vaccine management

o Clinical documentation

o Specimen handling

o Vital signs collection

o Clinical procedures

● Participate in the development and maintenance of clinic policies and procedures.

● Serve as a clinical resource for administrative and clinical team members.

Specimen Management and Diagnostic Testing

● Collect, label, package, and manage laboratory specimens.

● Coordinate specimen transport and laboratory submissions.

● Perform point-of-care testing as delegated and permitted.

● Ensure compliance with laboratory and diagnostic testing protocols.

● Monitor quality control procedures for clinic testing equipment.

Administrative and EMR Duties

● Utilize Electronic Medical Record (EMR) systems effectively.

● Document patient encounters and nursing activities accurately.

● Process laboratory, imaging, and consultation reports as directed.

● Assist with patient recalls and preventative care initiatives.

● Coordinate referrals and follow-up appointments as required.

● Support physician workflows and clinic efficiency initiatives.

Clinic Operations Support

● Monitor and restock clinical supplies.

● Assist with inventory management and supply ordering.

● Ensure examination rooms remain appropriately stocked and equipped.

● Assist with opening and closing clinic procedures.

● Participate in evening and Saturday clinic coverage as scheduled.

● Support clinic quality improvement initiatives.

● Contribute to a positive and collaborative team environment.

Qualifications

Required Qualifications

● Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse.

● Current professional liability protection.

● Minimum 2–3 years of nursing experience, preferably in primary care, family medicine, ambulatory care, community care, or a physician office setting.

● Experience performing clinical assessments and patient triage.

● Experience administering injections and immunizations.

● Experience obtaining and documenting vital signs.

● Experience working with Electronic Medical Record (EMR) systems.

● Strong communication, organizational, and interpersonal skills.

● Ability to work independently and collaboratively within a multidisciplinary team.

● Commitment to patient-centred care and continuous quality improvement.

Preferred Qualifications

● Family medicine or primary care experience.

● Experience with chronic disease management programs.

● Experience training and mentoring staff.

● Experience supporting quality improvement initiatives.

● Familiarity with Ontario preventative care programs and immunization schedules.

● Experience with Accuro, Avaros, OSCAR, PS Suite, or other EMR systems.

● Current CPR (Healthcare Provider) certification.

● Current First Aid certification.
